連續(xù)配戴軟性角膜接觸鏡對(duì)眼表上皮的影響
【摘要】 目的:探討軟性角膜接觸鏡(soft contact lens,SCL)連續(xù)配戴對(duì)眼表上皮的影響以及可能機(jī)制。方法:將SCL配戴者依據(jù)連續(xù)戴鏡時(shí)間分為<12h組、12~24h組和>24h組,共3組。對(duì)正常對(duì)照組(35例70眼)與病例組(99例198眼)進(jìn)行用角膜熒光素染色(fluorescein vital staining,F(xiàn)LS)、結(jié)膜印跡細(xì)胞學(xué)檢查(conjunctival impression cytology,CIC),對(duì)比各組間各觀察指標(biāo)差異。 結(jié)果:SCL戴鏡各組FLS記分、杯狀細(xì)胞密度與結(jié)膜上皮鱗狀化分級(jí)與正常對(duì)照組間差異有統(tǒng)計(jì)學(xué)意義(P<0.01);<12h組與12~24h組之間,<12h組與>24h組之間FLS、杯狀細(xì)胞密度與結(jié)膜上皮鱗狀化分級(jí)的差異有統(tǒng)計(jì)學(xué)意義(P<0.05);12~24h組與>24h組之間FLS、杯狀細(xì)胞密度與結(jié)膜上皮鱗狀化分級(jí)的差異無(wú)統(tǒng)計(jì)學(xué)意義。結(jié)論:連續(xù)配戴軟性角膜接觸鏡會(huì)導(dǎo)致眼表上皮損傷和結(jié)膜杯狀細(xì)胞減少,結(jié)膜上皮角化。且持續(xù)戴鏡時(shí)間越長(zhǎng),變化越明顯。

AbstractAIM: To observe the effects of extended soft contact lens(SCL) wear on the ocular superficial epithelial and analyze the possible mechanism.METHODS: The subjects were divided into three groups according to the wearing hours of SCL. Fluorescein vital staining( FLS), conjunctival impression cytology(CIC) were used in normal control group (35 cases 70 eyes) and experiment group (99 cases 198 eyes). The differences of FLS,goblet cell density(GCD)and grade of squamous metaplasia were analyzed.RESULTS: There were statistical differences in FLS,GCD and grade of squamous metaplasia between the group wearing SCL for less than 12 hours, 12 to 24 hours or more than 24 hours(P<0.05). However, there were no statistical differences in FLS, GCD and grade of squamous metaplasia between the group wearing SCL for 12 to 24 hours and more than 24 hours.CONCLUSION: Extended SCL wear can cause the ocular surface injury, decrease conjunctival goblet cells and conjunctival epithelium keratinized, and the longer the wear, the obvious the change.

我國(guó)約有500萬(wàn)配戴者,軟性角膜接觸鏡(soft contact lens,SCL)導(dǎo)致的眼部損害逐年上升。相當(dāng)一部分角膜接觸鏡配戴者并未嚴(yán)格遵守配戴處方規(guī)定,超時(shí)連續(xù)配戴現(xiàn)象普遍存在,導(dǎo)致近年來(lái)角膜接觸鏡引起的眼表并發(fā)癥明顯增加。對(duì)超時(shí)連續(xù)配戴SCL者眼表上皮的病理生理改變及發(fā)病機(jī)制的深入研究,有助于規(guī)范SCL的持續(xù)配戴和減少SCL連續(xù)超時(shí)配戴的危險(xiǎn)因素及并發(fā)癥。
1對(duì)象和方法中級(jí)職稱論文發(fā)表
1.1對(duì)象 病例組:年齡18~28(平均23.52±4.32)歲,因屈光不正配戴日戴型SCL,并能排除其他眼表疾病。依據(jù)SCL持續(xù)配戴時(shí)間<12h者36例72眼,12~24h者39例78眼,>24h者24例48眼。其中男45例90眼,女54例108眼,SCL持續(xù)配戴時(shí)間為8~360(平均62.2±79.7)h。正常組:年齡18~28(平均23.17±3.58)歲,其中男19例38眼,女16例32眼。SCL戴鏡組與正常對(duì)照組間年齡構(gòu)成和性別構(gòu)成差異無(wú)統(tǒng)計(jì)學(xué)意義。
1.2方法
1.2.1角膜熒光素鈉染色 采用熒光素鈉眼科檢測(cè)試紙(天津晶明新技術(shù)開發(fā)有限公司生產(chǎn))進(jìn)行活體角膜染色。

評(píng)分標(biāo)準(zhǔn)[1]:角膜表面分為4個(gè)象限,每個(gè)象限分別記0~3分(0分為無(wú)染色,1分為散在點(diǎn)狀染色或輕微劃痕,2分為中等量點(diǎn)狀染色并輕度融合,3分為密集點(diǎn)狀染色并融合。
1.2.2結(jié)膜印跡細(xì)胞學(xué)檢查 常規(guī)外眼消毒,4g/L鹽酸奧布卡因滴眼液(日本參天制藥株式會(huì)社)表面麻醉,硝酸纖維膜濾紙于上方球結(jié)膜表面細(xì)胞印跡取材,40g/L多聚甲醛固定液固定,PAS染色,蘇木素(Mayer)復(fù)染,二甲苯脫水,中性樹脂封片固定標(biāo)本。隨機(jī)計(jì)數(shù)100×光學(xué)顯微鏡5個(gè)0.5mm×0.5mm范圍內(nèi)杯狀細(xì)胞個(gè)數(shù)的平均值。根據(jù)Nelson分級(jí)標(biāo)準(zhǔn)[2]在400倍光學(xué)顯微鏡下觀察結(jié)膜上皮細(xì)胞大小、形態(tài)、核漿比,分為0級(jí)、1級(jí)、2級(jí)、3級(jí)。
統(tǒng)計(jì)學(xué)分析:對(duì)數(shù)據(jù)采用SPSS 18.0統(tǒng)計(jì)軟件進(jìn)行處理,計(jì)量數(shù)據(jù)用±s表示,首先進(jìn)行方差齊性檢驗(yàn)(F檢驗(yàn)),符合正態(tài)分布采用中均數(shù)的兩兩比較方差分析,結(jié)膜上皮細(xì)胞鱗狀化分級(jí)構(gòu)成比采用卡方檢驗(yàn),P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
2結(jié)果中級(jí)職稱論文發(fā)表
2.1角膜熒光染色計(jì)分 SCL連續(xù)配戴時(shí)間對(duì)角膜熒光染色(FLS)有影響(F=17.520,P<0.01)。連續(xù)配戴時(shí)間越長(zhǎng),角膜熒光染色計(jì)分越高(表1)。連續(xù)戴鏡時(shí)間<12h者影響較小;SCL持續(xù)配戴>12h者較<12h者FLS差異具統(tǒng)計(jì)學(xué)意義(P<0.01),但持續(xù)配戴12~24h與持續(xù)配戴>24h者之間相比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.140,表1)。表1 SCL持續(xù)戴鏡時(shí)間對(duì)角膜FLS計(jì)分的影響表2 SCL持續(xù)戴鏡時(shí)間對(duì)GCD的影響 表3 結(jié)膜上皮鱗狀化分級(jí)(Nelson法)構(gòu)成情況眼
2.2結(jié)膜杯狀細(xì)胞密度 連續(xù)配戴SCL時(shí)間對(duì)結(jié)膜杯狀細(xì)胞密度(GCD)有影響(F=44.918,P<0.01),各戴鏡組與正常對(duì)照組比較P值均<0.01,連續(xù)配戴12~24h與<12h組比較GCD有所減少(P<0.05),但>24h組與12~24h組GCD相比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P=0.334,表2)。隨著連續(xù)配戴時(shí)間增加,GCD減少明顯,但GCD的
